Please print out one copy of the needed form for each person attending therapy. Each person should complete his or her own form. If the client is a minor, please have him/her complete the form, but parent’s/guardian’s signatures are required on all signature lines. Please bring in completed forms to the beginning of the first session. Note: pages 6-8, titled “Notice of Privacy Practices” do not need to be given to the therapist, they are for the client’s records.
